FASTEST Acceptance Decision for Residential Treatment

Please provide the following to help connect your child to the best care possible!

  1. Demographic information to include name, date of birth, social security #, and physical address.
  2. Social history with demographic, family, and placement history.
  3. Reports or discharge summary from current/recent placements including specific information regarding their behaviors and reason for leaving.
  4. Most recent physical Exam.
  5. Most recent Psychiatric Evaluation (within last 3 months) with DSM5 Diagnosis.
  6. Most recent Psychological with IQ scores if one was completed.
  7. Most recent treatment information, progress notes, treatment plan etc.
  8. Educational information, IEP, school records (transcripts), grade level.
  9. Insurance information including subscriber’s name and date of birth.
  10. Referral source name and contact information.

Note: Additional materials will be requested prior to the actual admission date, which may include but not be limited to Birth Certificate, Immunizations Records, Court Order, Dental Records, Vision Screen.

Cumberland Hospital For Children And Adolescents

Residential program offers specialized services for youth ages 7 to 17. Patients with behavioral challenges receive medical supervision and care from a multidisciplinary staff, including a pediatrician, a child psychiatrist, therapists and nurses. The program goal is to help patients live more independently after discharge.

Referral checklist:

  • 7-17 years of age
  • Diagnosis of chronic medical illness, brain injury or neurological disorders co-occurring with behavioral diagnosis

  • Able to participate daily in 5.5 hours of school (psych)

Harbor Point Behavioral Center

Harbor Point Behavioral Health Center is a private residential treatment facility in Portsmouth, Virginia, that provides evidence-based care to children and adolescents with a primary DSM mental health diagnosis. Our programs provide a therapeutic and secure setting to address residents’ mental health needs and give them the tools needed to achieve a complete recovery.

The Hughes Center

Intensive (psychiatric) residential treatment and educational services for individuals with Intellectual/Developmental Disabilities (IDD/ASD) and co-occurring mental illness. Program features a multidisciplinary team approach to provide comprehensive evaluation and evidence-based treatment interventions to build upon the strengths and abilities of each resident within a secure “cottage-style” campus setting.

  • Highly structured treatment environment with specific emphasis placed on predictability, routines and visual schedules (principles of Applied Behavioral Analysis)
  • Functional Behavioral Assessments (FBA) and Behavioral Intervention Plans (BIP)

  • Individual is enrolled in an academic program of special education
  • Individual has failed at less intense levels of treatment within the community
  • Individual is medically stable
  • Impairment of the individual’s social, interpersonal, and/or communication skills requires the provision of moderate intensity, multidisciplinary treatment, integrated within a highly structured, 24-hour therapeutic residential setting.

Kempsville Center For Behavioral Health

Kempsville Center for Behavioral Health (KCBH) provides residential treatment services to children and adolescents, ages 11-17. Our trauma-informed, Cognitive Behavioral Therapy focused program provides individualized treatment and group processing in a secure environment. A skilled, multidisciplinary team works with families to develop a strong discharge plan for a positive transition back home. Remote teleservices are available. KCBH has national accreditation by The Joint Commission (JC), statewide accreditation by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) and educational accreditation and certification by Cognia™ (formerly known as AdvancEd™).TRICARE®-certified.

Intensive psychiatric residential treatment and special educational services for individuals with Intellectual/Developmental Disabilities (IDD/ASD) and co-occurring mental illness. The program is heavily focused on vocational exposure and skill-building. Liberty Point delivers residential treatment services in both highly secured and community-based environments based upon each youth’s adaptability and progress.

Secure Residential Treatment Center – Intensive psychiatric/behavioral health programs, trauma-informed, sexually reactive children, sexually abusive adolescent programs, substance use services, 60-90 day assessment. Residential Treatment Programs for children and adolescents at North Spring Behavioral Healthcare are designed for boys and girls ages 9 and older who may benefit from care in a secure, structured residential environment. POPLAR SPRINGS HOSPITAL

Poplar Springs Hospital is a 72-bed licensed by the Virginia Department of Behavioral Health and Developmental Services to treat both adolescent males and females ages 11-17 with significant emotional and behavioral disorders. TRICARE®-certified. Secure Residential Treatment Center: behavioral and mental health, co-occurring (mental health, substance use) treatment, trauma-informed environment, 30-60-90-day assessment and diagnostic.
